How do I know what out-of-pocket expenses I may have?
We will verify your insurance benefits and provide an estimate of the amount you may owe at your appointment. We collect 100% of all copays, coinsurance, and deductible amounts due.
Because your out-of-pocket responsibility is determined by your specific insurance plan, we strongly recommend contacting your insurance carrier directly to confirm your benefits prior to your appointment.
PLEASE NOTE: Arrowhead Endoscopy Center verifies benefits only for the facility fee.
You may receive additional bills from:
- Your physician
- The anesthesia provider
- Pathology services (if specimens are collected)
How Will My Insurance Be Billed?
As a courtesy, we will submit the claim to your insurance carrier on your behalf. Once the claim is processed, both you and Arrowhead Endoscopy Center will receive an Explanation of Benefits (EOB) from your insurance company.
- If the amount you owe is more than what was collected at your appointment, we will send you a bill for the remaining balance.
- If your insurance rejects the claim, we will attempt to resolve the issue and resubmit it.
- If the insurance carrier continues to reject the claim after these attempts, you may be responsible for any and all charges.
What If I Am Due a Refund?
After your claim is processed, the Explanation of Benefits (EOB) you receive from your insurance will show the amount you owe.
If the amount you owe is less than what you previously paid, we will issue a refund within 30–45 days, unless you request an earlier refund.
