INSURANCE & BILLING OVERVIEW

Arrowhead Endoscopy Center accepts most major insurance plans. As a courtesy, we will submit claims to your insurance carrier on your behalf. However, we are not able to determine what your specific plan will cover. Because your insurance policy is an agreement between you and your insurance company, we strongly encourage you to contact them prior to your procedure to verify your benefits.

Financial Responsibility

You are responsible for paying any copay, coinsurance, and/or deductible amounts before services are provided.

We accept:

  • Cash
  • All major credit/debit cards

We DO NOT accept checks or Care Credit.

After Your Insurance Processes the Claim

 Once your insurance company processes your claim, both you and Arrowhead Endoscopy Center will receive an Explanation of Benefits (EOB). This document will show:

  • What insurance paid to Arrowhead Endoscopy Center
  • The remaining balance you owe

If the amount you owe is greater than what was collected at the time of service, we will send you a statement for the additional amount due.
If the amount you owe is less than what was collected, we will issue a refund within 30–45 days, or sooner upon request.

Additional Billing

Arrowhead Endoscopy Center bills only for the facility fee.

You may receive separate bills for:

  • Physician fees
  • Anesthesia services
  • Pathology services (if specimens are collected)
