What is an Advance Directive?

An Advance Directive is a legal document that communicates your medical wishes in the event you are unable to speak for yourself.
Arizona laws governing Advance Directives are outlined in Arizona Statutes Title 36-3201 through 36-3210.

Advance Directives may include:

  • Living Will
  • Medical Power of Attorney
  • Pre-Hospital Medical Directive (DNR form)

Will Arrowhead Endoscopy Center Honor My Advance Directive

We will follow your Advance Directive to the best of our ability. However, we are required to inform you that:

  • We do NOT honor Do Not Resuscitate (DNR) orders.
  • In the event of a life-threatening emergency, we will perform all necessary resuscitate and stabilizing measures.

If you are transferred to a hospital for additional care, we will provide a copy of your Advance Directive to you and/or your medical transport team.

We also recommend registering your Advance Directive with the Arizona Healthcare Directives Registry (AZHDR) at azhdr.org before your procedure.

How Do I Provide an Advance Directive Before My Procedure?

f you already have an Advance Directive, you may bring a copy with you when you arrive for your appointment.

If you do not have an Advance Directive but would like to create one, you may complete the appropriate Arizona state forms prior to your procedure.

  • We cannot provide legal advice or guidance on how to complete these forms.
  • All forms are available at azhdr.org.
  • Upon request, we can provide you with a blank copy of the Arizona forms to complete on your own.
